This consumer-driven health plan covers you when you use doctors and hospitals in or out of our nationwide network. You can get care outside of our network, but your payments will be higher. The plan also includes a health reimbursement Arrangement (HRA) funded by SHBP. You can use your HRA to pay your eligible medical bills and annual deductible.
$125 Extra Benefit
You and your spouse can each earn up to $125 extra for your Health Reimbursement Arrangement by taking the Health
Assessment and getting an annual exam. Unused funds from your spending account are rolled over to use in the next plan year.
This traditional health plan covers you when you use doctors and hospitals in or out of our nationwide network. You can get care outside of our network, but your costs may be higher. There are no copayments on this plan. Instead, a simple deductible, coinsurance and out-of-pocket maximum apply. Prescription costs are applied to your deductible and coinsurance. This help keeps your out-of-pocket expenses low, compared to other plans, but still protects you from catastrophic health care costs.
HDHP 2010 Summary Plan Description
This traditional health plan covers you when you use doctors and hospitals in our nationwide network. This plan includes a high reimbursement level for your covered services from our network --- and no referrals are needed to see a network doctor. Except in true medical emergencies, services you receive from non-participating professionals are not covered. Under this plan, each covered family members is eligible for an annual $200 eyeglass or contact lens benefit regardless of where you purchase them.

Choice 2010 Summary Plan Description
The State Health Benefit Plan (SHBP) option, formerly referred to as your PPO plan, is now referred to by SHBP as an Open Access Plan (OAP) option. UnitedHealthcare calls our Open Access Plan option Choice Plus. UnitedHealthcare Choice Plus option functions the same as the PPO option by offering both in-network benefits and out-of-network benefits without the need for a primary care physician or a referral to see a specialist.
